Include a list of at least two references from public agency partners with full <br />names, contact information, and identification of the projects. <br />II. Project Site and Readiness— (25 points) <br />A. Project Site. Describe the proposed project site for the Homeless Shelter and <br />Navigation Center. Please make sure to include the following information: <br />1) Site location, size of location and size of building <br />2) Proximity to transit and community based resources <br />3) Site design as it relates to proposed use, target population and the surrounding <br />community. Include a Good Neighborhood Plan in the Exhibits. The Good <br />Neighbor Plan should include, but is not limited to, policies for community <br />involvement and neighborhood patrol, the safety and security of clients and the <br />surrounding neighborhood, mechanisms of communication, coordination and <br />complaint resolution with the surrounding neighborhood, business and public, local <br />police and fire departments, and maintenance of cleanliness in the neighborhood <br />surrounding the shelter. <br />A. Physical Condition of Site. Describe the physical condition of the site and any existing <br />structures. Include the following information: <br />1) Site control <br />2) Current zoning and any required zoning changes <br />3) Description of the physical improvements required to implement the proposed <br />project <br />4) Anticipated development costs, source of funds, and Contractor access to the <br />additional funds required for the project <br />5) Describe implementation plan that clearly states project readiness schedule. <br />III. Program Design and Outcomes — (25 points) <br />A. Program Description. Detailed narrative describing the proposed shelter and <br />explaining how the shelter design will ensure that multiple populations can be served. <br />Narrative should describe the program design and explain how the design will achieve <br />the desired outcomes. Please make sure to address the following information: <br />1) Continuum of Care System. Explain how the proposed shelter will be <br />integrated into the larger Continuum of Care system. <br />2) HMIS. Describe organization's experience working in HMIS and your plan to <br />comply with HMIS requirements. <br />3) Permanent Housing. Describe how clients will be connected to permanent <br />affordable housing through the use of rapid rehousing or other systems. <br />5B-42 21 <br />
