R <br />Ak <br />c. Program Staff is responsible for issuing the bus pass and completing the <br />information necessary on the bus pass log, the bus pass receipt form, and the <br />voluntary deduction agreement form, if applicable. <br />d. Copies of the bus pass receipt and Supportive Services Request Form (and <br />voluntary deduction agreement, if applicable) will be kept in the participant file. <br />2. Gas Cards <br />a. To receive a gas voucher, the following conditions must be met: The participant <br />must have a current California State Driver's License, have valid insurance, and <br />have the legal right to operate a vehicle involved. Copies will be placed in the <br />participant file. <br />b. Program Staff is responsible for informing the Program Coordinator the <br />participant is eligible to receive gas cards. A gas card request form must be <br />submitted with copies of California State Driver's License, insurance card, and <br />vehicle registration for initial gas card issuance. <br />c. One $50-dollar gas card will be issued per month. In the event the participant <br />loses the gas card, the participant will not receive a replacement. Exceptions will <br />be made at the discretion of the Program Manager. <br />d. Participants must agree to provide receipts from the gas station as proof the gas <br />card was utilized to pump gas. The receipts submitted must be dated the same <br />month the gas card was provided. The receipts submitted must be dated the <br />same month the gas card was provided and must total the exact amount of the <br />gas card issued. <br />e. Program Staff is responsible for issuing the gas card, making copies of the <br />receipts to place in grant files and for completing the information necessary on <br />the gas card log and the gas card receipt. <br />f. Copies of the gas card receipt and Supportive Services Request Form will be kept <br />in the participant file. <br />Other: <br />All supportive services under SAWIOA will be provided at the discretion of the Case Manager <br />and/or Program Coordinator.
