City of Santa Ana <br />RFP: Year -Round Homeless Shelter and Navigation Center <br />VII. Project Sustainability — (10 points) <br />A. Narrative description addressing the sustainability of the project over a ten-year <br />period. <br />Vill. Required Exhibits and Attachments <br />Respondent shall complete and return the following Exhibits attached herewith: <br />• Exhibit A - Contractor's Statement <br />• Exhibit B - Additional Insured Endorsement for Commercial General Liability Policy <br />(Not required until after contractor selection) <br />• Exhibit C - Certification of Nondiscrimination <br />• Exhibit D - Non -Collusion Affidavit <br />• Exhibit E - Commitment to Enter into Memorandum of Understanding with the Santa <br />Ana WORK Center <br />Respondent shall submit the following attachments: <br />• 501 c(3) letter and evidence in good standing for each partner <br />• Three years of audited financial statements (including management letters) <br />• Minimum of two (2) references associated with current homeless shelter contracts <br />• Memorandums of Understanding (MOU) with Third Party Organizations (if applicable) <br />• Business Plan which includes an Operational Pro -Forma for the Shelter Operation <br />(10 years) <br />• Preliminary Project Pro -Forma including Pre -Development Costs <br />• Preliminary Sources and Uses Statement <br />• Organizational Chart, including key staffing for the proposed project <br />• Staff Resumes & Job Descriptions <br />• Site Map <br />• Documentation of Site Control <br />• Site Preparedness and Permitting Plan <br />• Individual Service Plan (Draft) <br />• Management and Operations Plan (Draft) <br />• Program Policies & Procedures (Draft) <br />• Good Neighbor Plan (Draft) <br />• Shelter Client Rules (Draft) <br />• Letters of commitment for services <br />• Third -Party Site Feasibility Assessment (if applicable) <br />Please include any additional information you deem essential to a proper evaluation of your <br />proposal and which is not solicited in any of the preceding sections. Creative and innovative <br />ideas can be addressed through -out the proposal.
