VII. Required Information/Documents to Qualify for Grant Award: <br />• Business License <br />• Certificate of Occupancy <br />• Bank Statements for February 2020 and May 2020 (additional bank statements may <br />be requested) <br />• Statement showing planned use of funds <br />• Employee list as of February 2020 and Employee list as of May 2020 <br />VIII. Project Expectations: <br />The CONTRACTOR will implement a rapid deployment small business grant program to <br />address the challenges faced by small enterprises located in the CITY. The <br />CONTRACTOR will: <br />1. Work with the CITY, the Orange County Small Business Development Center, Santa Ana <br />Chamber of Commerce, and other small business support organizations in the CITY to <br />promote and market the program so that as many small business owners as possible <br />throughout the CITY know about the program and are able to apply. <br />2. Develop and host a user friendly online application and affidavit for the grant program with <br />a public link which will direct to an on-line application. The landing page for the <br />application will describe the program and invite eligible applicants to apply. All application <br />responses will be securely stored in an electronic file system, which will serve as a <br />repository for all program documents, and available for CITY staff to view at any time. <br />The online application system to be used is Submittable. <br />3. CONTRACTOR will develop a draft press release and a e-flyer for the program. <br />4. Online applications will be available in English, Spanish and Vietnamese. <br />5. Once grant inquiry applications are received, CONTRACTOR will determine the initial <br />eligibility status of the business. CONTRACTOR will also check against lists (some of <br />which the CITY will provide others will be provided by the CONTRACTOR) containing <br />the data of ineligible businesses (i.e. previous funding as DI or City of Santa Ana grantees, <br />businesses with active code violations) to confirm eligibility. Once approved for funding, <br />grantees will enter the award phase whereby additional documents will be collected (ACH, <br />W9, valid identification and Grant Agreement in DocuSign). <br />6. Once the grantee has completed all required documentation in the grant award process, <br />underwriting will take place. CONTRACTOR Accounting Staff will work with City of <br />Santa Ana staff to approve grantees and fund ACH payments. <br />7. Ineligible business grant applications will be notified of Ineligible status. <br />20 <br />
