Disbursement of Funds: <br />Individual, business, and non-profit awardees will receive the approved funding in one <br />disbursement and after submitting required documents. <br />VI. Required Documents to Oualifv for Grant Award: <br />• Grant Agreement/Application Affidavit <br />• Two Bank Statements: April 2019 monthly bank statement and April 2020 <br />monthly bank statement <br />• Two Employee Lists (for businesses and non -profits): One from February 29, <br />2020 and one from April 30, 2020 <br />• Statement describing grant fund use <br />• W-9 form <br />• CONTRACTOR may request additional information as they see fit to determine <br />eligibility <br />VII. Project Expectations: <br />The CONTRACTOR will implement a rapid deployment of arts relief grant program to <br />address the challenges faced by arts -related entities located in the CITY. The <br />CONTRACTOR will: <br />Work with the CITY and arts support organizations to promote and market the <br />program so that as many individual artists, arts -related micro -business owners, and <br />arts -related non-profit organizations as possible throughout the CITY know about the <br />program and are able to apply. <br />2. Launch a rapid deployment application (English) process through the existing OC <br />Arts & Culture Resilience Fund, a relief fund housed at Charitable Ventures, for the <br />grant program and a public link which will direct to an on-line application on both the <br />CONTRACTOR'S web site as well as the CITY'S web site. The landing page for the <br />application will describe the program and invite eligible applicants to apply. All <br />application responses will be securely stored in a repository for all program <br />documents, and available for CITY staff to view at any time. <br />18 <br />
