EXHIBIT A SCOPE OF WORK The Contractor agrees to provide all labor, equipment, and materials necessary to operate and manage one (1) stationary FDA/CDC approved covid-19 testing site located at Santa Ana College and community testing sites selected by the City. The goal is to test at least 32,000 individuals in the City of Santa Ana within an eight (8) week period beginning August 18, 2020. Testing will be limited to residents of Santa Ana, and/or students currently enrolled in Santa Ana College or any other post -secondary college, university, or trade school with a campus in Santa Ana. Proof of residency or college enrollment will be required. The City has identified one (1) stationary testing site in the City of Santa Ana, California. The site selected is based on the size and logistical requirements of the testing site and convenience from all areas in the City of Santa Ana. Santa Ana College — 1530 West 17th Street, Santa Ana, California 92706 Site Operations Testing site hours: Days and times shall be determined by Contractor in collaboration with the City of Santa Ana. Testing will be by appointment only. Masks are required and will be provided, if necessary. Social distancing will be maintained throughout the process and those arriving by vehicle for testing will be asked to remain in their vehicles at all times. Individuals arriving at test sites on foot will be directed to a dedicated and protected testing area. Testing shall include one (1) CDC/FDA approved self -testing kit per person with a cotton swab applicator. Self -testing kits are used by both children and adults. Any child under the age of eighteen (18) years old must be accompanied by a parent or guardian. Parent or guardian must administer the test to any child under twelve years (12) years of age. Test kits are handed to each individual and a medically trained technician provided by the Contractor will be available to explain how to use it. Individuals taking the test DO NOT need to exit their vehicles. Self- tests can be taken inside the vehicle. Once the individual completes the self -test, the test swab will be returned to the medical technician, hermetically sealed, and sent to the testing laboratory for processing. Test results will be available in 48-72 hours. All individuals tested will be notified of the results. Individuals with a positive covid-19 test result will be immediately notified in a confidential manner by the Contractor's medical director and requested to self -quarantine and/or seek medical advice. 18 4y_V
