Individuals who have completed the testing process and have received the results may request a <br />certificate of completion for employers or back -to -school purposes. Certificates will be made <br />available by the Contractor's medical director. <br />Contractor agrees to provide City with a weekly report detailing the activities of the week, <br />including number of tests taken by site, number of tests processed by each laboratory, including <br />test results (positive and negative), and any other information requested by the City. Contractor's <br />state licensed medical directors will administer all testing and medical related activities. <br />The Contractor has partnered with the following CDC/FDA approved medical laboratories for <br />covid-19 testing: <br />Fulgent Genetics — 4978 Santa Anita Avenue, Temple City, California 91780 <br />Balboa DX Diagnostic Laboratories — 1101 Bayside Drive, Corona del Mar, California <br />92625 Stationary Site <br />The Contractor and City agree that said Scope of Work will be conducted weekly at Santa Ana <br />College in order to reach its goals and meet the eight (8) week timeline. <br />Work Start: Saturday, August 22, 2020 <br />Work Frequency: Each Saturday and Sunday for eight (8) weeks <br />Testing Hours: Testing from 9:00 a.m. to 4 p.m. with set-up one hour prior and tear -down <br />one hour following <br />Mobile Sites <br />The Contractor and City, at the City's direction, agree that said Scope of Work will be conducted <br />daily, Monday through Friday, at sites selected by the City beginning August 18, 2020. Logistics <br />at each site may vary and will be at the direction of the City, provided they follow CDC guidelines <br />and recommendations by Contractor's medical directors. All appointments not made on the <br />website will be handled directly by the City. <br />Work Start: Tuesday, August 18, 2020 <br />Work Frequency: Monday through Friday for approximately eight (8) weeks with a maximum <br />of eight hour per day. <br />Completion <br />Expected Work Completion: Saturday, October 10, 2020 or until 32,000 tests are completed. Any <br />delays or cause for delay that arise during the course of the project will be immediately reported <br />to the City. <br />Project Staffing <br />The Contractor agrees to provide the following staff to sufficiently meet the needs of the overall <br />project and reduce any waiting time and/or un-necessary social contact while testing is in progress, <br />19 1- <br />
