Categories: <br />Project Coordinator 1 <br />Contact Tracing 25 <br />TOTAL 26 <br />Payment/Invoicing <br />Contractor shall submit weekly invoices to the City for services rendered and for the tests performed. <br />Contractor will be billed at the mutually agreed rate of $140 per test. <br />Invoices shall be supported by a weekly activity report, which is itemized and detailed by test site. <br />City must notify the Contractor of any challenges or disputes to the invoice within five (5) working <br />days of City receiving the invoice. <br />Cost for each Contact Trace is $350 or 1429 investigations for the prescribed allowance of$500,000. <br />Cost for Contract Tracing shall be invoiced weekly to the City but separate of the invoice for covid- <br />19 testing. Invoices shall include a summary report for all investigations performed. <br />Payment for each invoice (2) is due net 10-days. <br />Changes/Modifications <br />City and Contractor agree that both parties can make no changes to this Agreement without the <br />express approval by both parties in writing. <br />Subcontractors <br />City agrees that the Contractor may hire subcontractors at its discretion, provided that Contractor <br />agrees that the payment to said subcontractors is entirely the Contractor's responsibility. Contractor <br />agrees to provide City with an updated list of any subcontractors used in the performance of therefore <br />mentioned Scope of Work. <br />Site Management <br />At the end of each workday, Contractor agrees to secure all testing materials and equipment using a <br />secure container. Contractor shall be responsible for the contents in the container. Contractor shall <br />also secure and remove all trash generated by Contractor from the test site on a daily basis. <br />Contractor shall also work with the City to ensure that restroom facilities are available and that they <br />remain safe and clean for staff at all times. <br />At the completion of the project, Contractor shall ensure the testing site be returned to the state in <br />which it was found prior to the beginning of the project. <br />Website <br />Contractor will design and manage a website (in three languages) for the project. Said website will <br />be updated weekly as to test locations and any other pertinent information as it relates to the covid- <br />22 <br />
