EXHIBIT A SCOPE OF WORK ATTACHMENT A EY�ZIIii-D[I70lriiliJC� C1 Santa Ana CARES Child Care Grant AUGUST 1, 2020 - DECEMBER 31, 2020 The Santa Ana Child Care Payment Assistance program is designed to assist households within the City of Santa Ana that have been affected by COVID-19. The funding, which is part of the CARES Act Coronavirus Relief Funds bill, will aid households impacted by COVID-19 that need child care to stay on the.job or get back to work. Funding will be allocated in the form of grants, on a first come first serve basis. CONTRACTOR shall develop, implement and administer the Santa Ana CARES Child Care Grant pursuant to this scope of work and the terms of this agreement. I. Size of the grant: The Santa Ana CARES Child Care Grants will provide up to $1000 per eligible Santa Ana Household. The Grant is up to $100 per week, irrespective of the number of children in the Household, up to a maximum total of$1000 per Household. Households will be limited to one grant. Grant recipients must be able to provide a certified statement that funds received were spent in accordance with program requirements through the program application. II. Households must have been affected by COVID-19: The household must submit documentation that it is impacted by COVID-19 in one of the following ways: • Job loss as a result of COVID-19 (from the time period of March 1, 2020 through present) • Reduction in work hours leading to a decrease in household income. • Is an essential worker and in need ofchildeare in order to keep working due to COVID- 19. • Demonstrated need must be documented and verified as outlined in Exhibit A Section VII. IIL Eligibility Requirements: Households must meet the following eligibility requirements: • Must live in Santa Ana. • Child(ren) must live in Household and be under the age of 13 18
