Eligible Expenses of CARES Act Grant: <br />Households can use the funding on the following eligible expenses: <br />• Cost of Child Care for Children Under the Age of 13. <br />V. Disbursement of Funds: <br />Eligible Households will receive the approved funding in one disbursement via check. <br />VI. First Come First Serve Basis: <br />Applications will be reviewed and approved on a first come First serve basis. <br />VII. Required Information/Documents to Qualify for Grant Award: <br />• Proof of Santa Ana Residency <br />o Government Issued Identification or Driver's License OR <br />o Utility Bill/Other Bill <br />• Copy Identification Card of Applicant <br />a Government Issued Identification or Driver's License OR <br />o Matricula Consular OR <br />a Passport <br />• Proof of relationship between applicant and children <br />o Child Birth Certificate OR <br />o Proof of Guardianship <br />• Letter from Employer <br />o Documenting job loss OR <br />o Reduction in work hours OR <br />o Documenting Essential Worker <br />o Self -Attestation <br />• To be used as a last resort, provided original source documentation is not <br />available to document job loss, reduction in hours or essential worker <br />status. <br />• Certified Statement showing planned use of funds <br />VIII. Project Expectations: <br />The CONTRACTOR will implement a rapid deployment child care grant program to <br />address the challenges faced by families residing in the CITY.
