PROPOSAL CONTENTS <br />Proposals are to be prepared in such a way as to provide a straightforward, concise <br />delineation of capabilities to satisfy the requirements of this RFP. Colored displays, <br />promotional materials, etc., are not necessary or desired. Emphasis should be <br />concentrated on conformance to RFP instructions, responsiveness to the RFP <br />requirements, and on completeness and clarity of content. Dividers and clear organization <br />of content and material are encouraged. <br />1. Cover Letter <br />Proposals shall include a letter signed by a principal or authorized representative who <br />can make legally binding commitments for the entity. Include type of business entity. <br />Cover Letter shall not exceed one page. <br />Cover letter must be addressed to the following: <br />Jason Kwak, Building Safety Manager <br />City of Santa Ana — Planning and Building Agency <br />20 Civic Center Plaza <br />Santa Ana, CA 92701 <br />2. Agreement Statement <br />Proposal shall include a statement outlining your concurrence or concerns with any and <br />all provisions as contained in the Standard Agreement attached as as Exhibit A of this <br />RFP. <br />3. Firm and Team Experience <br />Proposal shall include a profile of the firm's experience including the following: <br />i. A general description of the firm, including size, number of employees, <br />number of credentialed actuaries with their affiliations, and any past or <br />contemplated changes in ownership. <br />ii. Name and contact information of the supervising associate to be assigned <br />to the engagement. <br />iii. Resumes for the professional staff assigned to the engagement. <br />iv. A description of the firm's experience in providing similar services to those <br />requested in this RFP. <br />V. A list of the local office's most significant engagements in the last five (5) <br />years, indicating whether they are public or private sector, and including <br />scope of work, date, supervising actuaries, and name and telephone <br />number of the client contact. <br />4. Cost Proposal <br />All Proposers are required to submit a cost breakdown by contract year (including <br />renewal option term if exercised); including estimated hours, hourly rates, expenses, <br />and a total not -to -exceed cost with their Proposal. Pricing instructions should be clearly <br />defined to ensure fees proposed can be compared and evaluated. Proposals shall be <br />valid for a minimum of ninety (90) days following Proposal deadline. The cost for <br />developing the Proposal is the sole responsibility of the Proposer.
