REPORTS, AUDITS AND ACCOUNTABILITY <br />Proposals should include samples of the following Contractor's reports to the Jail Administrator <br />and any other reports that may be useful. <br />Sales and commission reports should indicate: sales total for order forms, and commission due <br />the Inmate Welfare Fund. Each of these reports should be produced monthly with an annual <br />report as well. <br />E. PROPOSAL SUBMISSION <br />1. The proposer shall provide a listing of all items that can be sold through commissary and <br />proposed sale prices for each item. <br />2. The Proposal shall indicate the percentage of net sales to be paid to the City each month. <br />"Net Sales" is defined as total of all sales, minus sales tax and sales of U.S, postage <br />stamps. <br />F. SECURITY <br />The following security requirements shall be in effect at all times. Failure to abide by any of <br />these items by any Contractor's employee may result in removal from the facility and or criminal <br />charges, <br />1. SECURITY SCREENING <br />a. All Contractor's personnel to work within the facility must successfully complete a <br />background investigation conducted by the Santa Ana Police Department before <br />beginning work. Contractor employees that do not successfully complete the <br />background investigation shall not be allowed entry into the facility. No less than 45 <br />days before an employee is scheduled to begin work, the Contractor shall submit to <br />the Chief of Police the following information for each employee to work within the <br />facility. <br />• First name, middle name, last name <br />• Date of birth <br />• Current resident address <br />• Photocopy of valid government Issued photo identification <br />+ Photocopy of valid Social Security card <br />• Proof of U.S. citizenship or legal work status <br />b. The Contractor shall be notified of any employees that are refused for employment <br />within the facility. The background, investigation requirement shall also apply to <br />employees who begin at any later date. <br />c. Decisions of the Santa Ana Police Department are final and are not subject to <br />arbitration.
