Exhibit 1 <br />c) References: Proposal shall include a listing of relevant projects with <br />references for three public entities for which Proposer has performed similar <br />work within the past five (5) years. <br />4. SCOPE OF WORK AND UNDERSTANDING OF NEED: <br />a) Scope of Work: Proposal shall include a Scope of Work which details the <br />work phases to be completed, the tasks to be accomplished, and the <br />deliverables to be provided based on the requested Scope of Work detailed <br />in Attachment 1 of this RFP. <br />b) Understanding of Need: Proposal shall include narrative which <br />demonstrates the firm's understanding of the work. This should include <br />your firm's philosophy, anticipated approach, methodology, tasks <br />necessary for successful completion, deliverables, and suggestions or <br />special concerns that the City should be made aware of. Identify any <br />assumptions and/or exclusions used in preparation of the scope of work <br />and associated fee estimate. Include a description in your proposal of your <br />proposed public input process and your expected outcomes from each <br />meeting. <br />5. SCHEDULE: Proposal shall include a Schedule which details the work phases <br />to be completed, the tasks to be accomplished, and the deliverables to be provided <br />based on the requested Scope of Work detailed in Attachment 1 of this RFP. <br />6. FEE PROPOSAL: The fee proposal shall be submitted concurrently with the <br />technical proposal and shall correspond to tasks as outlined in the scope of Work. <br />The fee proposal shall include line items for services that are all inclusive and shall <br />incorporate any potential costs related to meals, travel, lodging, or other such items <br />that are normally considered reimbursable and not delineated as a separate line <br />item. <br />Proposal shall also include the firm's Standard Hourly Fee Schedule and a <br />sample invoice. <br />7. CERTIFICATIONS/STATEMENTS: <br />The following forms, included in Attachment 3 in the Appendix of this RFP shall be <br />signed and included as part of the proposal submittal package: <br />Attachment 3-1: Non -Collusion Affidavit <br />Attachment 3-2: Non -Lobbying Certification <br />Attachment 3-3: Non -Discrimination Certification <br />Attachment 3-4: Agreement Statement <br />
