Contractor <br />You are hereby directed to make the herein changes from the plans and specifications or do the following described work not included in the plans and <br />specifications on this contract. <br />NOTE: THIS CHANGE ORDER IS NOT EFFECTIVE UNTIL APPROVED BY THE CITY COUNCILOR CITY MANAGER. <br />Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. <br />Change requested by PUBLIC WORKS AGENCY <br />EXTRA WORK AT TIME & MATERIALS <br />1) To compensate the contractor for costs associated with the 16-inch valve replacement at Station <br />23+50 on September 13, 2021(PCO No. 12). <br />TOTAL COST = $4,735.28 <br />2) To compensate the contractor for costs associated with the 12-inch hot tap at Station 28+25 on <br />October 29, 2021 (PCO No. 5). <br />TOTAL COST = $3,539.94 <br />3) To compensate the contractor for costs associated with the 12-inch hot tap at Station 28+25 on <br />November 1, 2021 (PCO No. 5). <br />TOTAL COST = $2,628.47 <br />4) To compensate the contractor for costs associated with the tie-in and backfill at Station 28+25 on <br />November 2, 2021 (PCO No. 17). <br />TOTAL COST = $6,849.29 <br />5) To compensate the contractor for costs associated with 4 additional feet of pipe at Station 28+25 <br />on November 3, 2021. <br />TOTAL COST = $1,936.58 <br />6) To compensate the contractor for costs associated with valve removal and blind flange per RFI#1 <br />on November 8, 2021 (PCO No. 3). <br />TOTAL COST = $2,345.80 <br />7) To compensate the contractor for costs associated with abandoning water services along Grand <br />Avenue from Station 11+00 through 16+00 on November 9, 2021 (PCO No. 4). <br />TOTAL COST = $2, 800.00 <br />8) To compensate the contractor for costs associated with abandoning water services along Grand <br />Avenue from Station 11+00 through Station 16+00 on November 10, 2021 (PCO No. 4). <br />TOTAL COST = $3,000.00 <br />Page 1 of 3 <br />
