8.3 Reports <br />8.3.1 Report Formats and Schedule <br />Records shall be maintained in forms and by methods that facilitate flexible use of data <br />contained in them to structure reports, as needed. Reports are intended to compile <br />recorded data into useful forms of information that can be used to, among other things: <br />a) Determine and set rates and evaluate the efficiency of operations; <br />b) Evaluate past and expected progress toward achieving AB 939 goals and <br />objectives; <br />c) Determine needs for adjustment to programs; and, <br />d) Evaluate Customer service and complaints. <br />Contractor may propose report formats that are responsive to the objectives and <br />audiences for each report. The format of each report requires approval by City. The <br />Contractor agrees to submit all reports by electronic means in a format compatible with <br />City's software/computers at no additional charge, if requested by City. Contractor <br />will provide a certification statement, under penalty of perjury, by an authorized <br />Contractor official, that the report being submitted is true and correct. <br />If requested, Contractor's complaint summary, shall be sent to the Executive Director <br />within five days of request. Reports are to be submitted in Microsoft Excel, or another <br />software as requested by City and shall be submitted electronically to City, as directed, <br />and to: <br />Executive Director (or designated representative) <br />Public Works Agency <br />City of Santa Ana <br />20 Civic Center, Plaza M-21 <br />Santa Ana, CA 92701 <br />8.3.2 Monthly Reports <br />Monthly reports shall be submitted within 30 calendar days after the end of each <br />month. <br />The information listed shall be the minimum reported: <br />August 17, 2021 127 City of Santa Ana <br />
