number, demonstrating that the required visits have been made, and reason <br />provided for not establishing a recycling or organics program, in accordance <br />with Sections 4.3.5.1 and 4.3.6.1. The site visit summary will be completed on <br />forms approved by the City. <br />3. List of Customers that are required to participate in an Organics Recycling <br />program per Public Resources Code Section 42649.81. <br />4. Documentation of the universal service enrollment process, including a copy of <br />the City-wide Customer enrollment level evaluation conducted pursuant to <br />Section 5.8of this Agreement. <br />5. List of all Commercial and Multi -Family Premises Customers with a Solid Waste <br />service level of two (2) cubic yards of service capacity per week or more. Such list <br />shall include each such Customer's service address and Refuse, Source Separated <br />Recyclable Materials, and Source Separated Organic Materials service levels. <br />6. The total number of de minimis waivers, and physical space constraint waivers <br />granted in the quarter (by month), if any, including the Customer name and <br />address for each waiver. <br />The number of waiver reverifications performed by the Contractor pursuant to <br />Section 5.6.13 of this Agreement in the quarter (by month), if any, including a <br />copy of documentation for each reverification inspection, which shall include, at <br />a minimum: the Customer's name, address, and Customer type; the type of <br />waiver being verified; any photographic or other evidence collected during the <br />inspection; and the resulting recommended conclusion by the Contractor <br />regarding the validity of the waiver. The Contractor shall provide a summary of <br />recommendations to the City of all waivers which the Contractor concludes to no <br />longer be warranted. <br />8. List of Commercial and Multi -Family Premises Customers that do and do not <br />participate in an Organics program, whether the Organics program is provided <br />by Contractor or another party, and whether the program is for Food Waste <br />and/or Yard Waste (such as a landscaper that composts or otherwise diverts <br />Organic Materials). <br />9. Commercial and Multi -Family Customers participating in food recovery <br />programs. <br />10.
