The goal of HEAL-OC is to address inequities related to COVID-19 and overall health literacy in <br />surrounding communities with high social vulnerability index (SVI) scores. The goal will be met <br />through attainment of four objectives: (1) developing community -based partnerships, including <br />establishing a Community Stewards Board, (2) developing a Disparities Impact Statement as a <br />framework for intervention and evaluation, (3) developing and implementing a Health Literacy Plan in <br />adherence with National CLAS Standards and (4) conducting rigorous and timely Evaluation and <br />Quality Improvement. <br />O LJh t MI ri V 11 fi,'�',°u <br />By achieving these four objectives, HEAL-OC aims to realize the following six objectives: <br />Outcome 1: Capacity <br />1.1 In the first 3 months, co -develop with community partners and stakeholders a Disparity Impact <br />Statement as a framework to guide the development, implementation and evaluation of a Population <br />Health Literacy Plan. <br />Outcome 2: COVID 19 Education <br />2.1 In the first 6 months, develop and implement a Population Health Literacy Plan to deliver <br />linguistically and culturally appropriate (i.e. aligned with National CLAS Standards) COVID-19 <br />education and resources to Latino and Asian American and Pacific Islander (AAPI) residents in the <br />targeted communities. <br />2.2 By the end of the project period, deliver education to residents in the targeted communities. <br />Outcome 3: COVID-19 Services <br />3.1 In the first 6 months, achieve a 25% gap reduction in COVID-19 testing and vaccination access <br />for 4th quartile SVI areas, predominantly in Santa Ana and surrounding areas, compared to the rest <br />of the County. <br />3.2 By the end of the project period, achieve a 50% gap reduction, and achieve a 50% increase in the <br />proportion of residents with a confirmed vaccination in an information system (HP2030 measure <br />I I D-D02). <br />
