CONTRACTOR shall provide this record <br />to ADMINISTRATOR upon request. <br />11.8 Participate in the evaluation of services through distribution and collection of <br />customer satisfaction surveys as determined by ADMINISTRATOR. Survey <br />results, along with any corrective action(s) with respect to survey findings or trends, <br />shall be provided to ADMINISTRATOR upon request. <br />11.9 Maintain a monthly Complaint Log documenting PARTICIPANTS' complaints <br />and CONTRACTOR's response. The Complaint Log shall be submitted to <br />ADMINISTRATOR by the tenth (1 Oth) calendar day of each month. Any incidents <br />involving harassment or injury must be reported to ADMINISTRATOR within one <br />(1) business day of the incident. <br />11.10 Maintain an organizational chart along with corresponding job descriptions for <br />every classification funded under this Agreement. CONTRACTOR shall provide <br />an updated organizational chart along with job descriptions to ADMINISTRATOR <br />by July 1, 2022. <br />11.10.1 Should any organizational or staffing arrangements change during the <br />term of the Agreement, CONTRACTOR shall submit a revised <br />organizational chart to ADMINISTRATOR within two (2) business days; <br />or upon request by ADMINISTRATOR. <br />11.11 Provide additional reports as requested by ADMINISTRATOR. <br />12.
