11.1 Comply with data verification requirements per ADMINISTRATOR. <br />11.2 Immediately implement and adhere to future policy revisions and changes or <br />enhancements in procedures and processes pertaining to submission and reporting <br />requirements. <br />11.3 Use the most current templates provided by ADMINISTRATOR. <br />11.4 Implement a quality control process to ensure integrity of data submitted, review <br />and approve all reports submitted to ADMINISTRATOR. <br />11.5 Provide by the tenth (1 Oth) calendar day of each month, a status report for the <br />preceding month, in a format approved by ADMINISTRATOR. Status reports <br />shall include, but are not limited to, the following: <br />11.5.1 Total number of referrals received from ADMINISTRATOR and referral <br />outcomes. <br />11.5.2 Caseload movement: total number of PARTICIPANTS enrolled, <br />participating, exiting, and completing job placements or programs. <br />11.5.3 Total number of referrals returned to ADMINISTRATOR prior to <br />placement. <br />11.5.4 List of PARTICIPANTS who obtained unsubsidized employment within <br />forty-five (45) calendar days of completing the activity, or otherwise <br />agreed timeline requested by ADMINISTRATOR. <br />11.5.5 Updated information on the activity status of the referrals and <br />PARTICIPANT attendance issues. <br />11.5.6 List of issued incentive vouchers including PARTICIPANT name, <br />issuance date, and any related information requested by <br />ADMINISTRATOR. <br />11.6 Provide monthly PARTICIPANT attendance and progress reports including <br />achievements and the number of participation/work hours to ADMINISTRATOR, <br />as applicable, by the tenth (1 Oth) calendar day of the following month. <br />11.7 Maintain a record of all WEX worksites who have served PARTICIPANTS and <br />include the percentage of PARTICIPANTS who were offered unsubsidized <br />WQMO.522 Page 13 of23 March 1, 2022 <br />City Council 20 — 60 3/15/2022 <br />
