EXHIBIT A <br /> <br />Coalition of Orange County Community Health Centers <br />Project Narrative and Workplan for OMH HEALS Grant <br />Project Narrative <br />As part of the OMH Health Equity and Literacy Project- OC Grant- The Coalition of Orange County Community Health Centers proposes to work <br />with the OC Health Equity COVID-19 Community-Academic Partnership to identify effective approaches for improving health outcomes with the <br />ultimate goal of promoting dissemination, adoption and sustainability of these approaches. The focus of this project will be to address health <br />alition hopes to <br />identify and test strategies and interventions that will improve access to accurate and actional health information; and thereby improving the <br />accessibility, quality, and safety of health care; and improve the health and quality of life for the vulnerable populations across Orange County. <br />As part of this larger partnership, the Coalition of Orange County Community Health Centers will focus on the following goals: objectives; and <br />activities as part of the quality improvement portion of the project over the 24-month grant period: <br />Goal: To improve health literacy; reduce gaps in health disparities for low income, medically underserved patients at Federally Qualified Health <br />Centers and other Community Clinics in the highest SVI areas in Orange County. <br />Outcome: Increase COVID-19 testing and vaccination and reduce and/or prevent COVID-19 for low income, medically underserved patients <br />(serving over 10000 unique patients- across four health centers). <br /> <br />Objective Activities Outcome Staff (Role) <br />1.
