The Crossroads at Washington Prepared March 11, 2022 <br />Washington Santa Ana Housing Partners, L.P. <br />• Case Manager — responsibilities include, but are not limited to, providing individual case <br />management referrals and supportive services for tenants; process housing applications <br />using the Coordinated Entry System ("CES"), Family Solutions Center ("FSC"), and <br />interview potential tenants for permanent supportive housing; and developing Individual <br />Service Plans in collaboration with tenants. <br />Please note that because the project contains less than seventy-five percent (75%) Special Needs <br />units, and is fifty percent (50%) Special Needs and fifty -percent (50%) Large Family, services <br />are provided proportionally for each population type. <br />In addition to the services provided by ACOF, the Orange County Health Care Agency <br />("OCHCA") will provide individualized services for the twenty (20) Mental Health Services Act <br />tenants. <br />Project Population Type <br />As described above, the Project consists of eighty-six (86) units. Of the eighty (86) units, forty- <br />three (43) units are dedicated for permanent supportive housing and one (1) unit dedicated as <br />Manager's unit. The "for profit" administrative general partner is <br />Related/Washington Santa Ana Development Co., LLC, an affiliate of Related. The "not for <br />profit" managing general partner will be Supportive Housing LLC, whose member is A <br />Community of Friends. The John Stewart Company ("JSCo") will manage the Project. <br />Related/Washington Santa Ana Development Co, LLC, will own a .0075% interest and <br />Supportive Housing LLC will own a .0025% interest and act as the Managing General Partner. <br />
