AESCO <br />AESCO Proposal No. P6513 <br />Statement of Qualifications <br />Section B: Contract Agreement Statement <br />AESCO has reviewed the City of Santa Ana Sample Consultant Agreement and we accept all <br />of the provisions as contained within the Agreement in Attachment 2 of the Appendix. <br />Section C: Firm and Team Experience <br />AESCO is a woman -owned corporation (incorporated in California) and has been in <br />operation since 1993. AESCO is headquartered at 17782 Georgetown Lane, Huntington <br />Beach, California 92647 and we currently have thirty-five employees. We also have a second <br />office in Fontana, California. Mr. Adam Chamaa, P.E., G.E. will be the single point of <br />contact for this proposal and he can be reached at 714-375-3830 or emailed at <br />Adam.ChamaagAescotech.com. AESCO is SBE, DBE/UDBE, WBE, and CBE certified. <br />AESCO's laboratory is certified by DSA, Caltrans and City of Los Angeles. AESCO has <br />been involved in a number of projects involving QA/QC construction materials testing and <br />inspection and geotechnical engineering for various clients including the Cities of Los <br />Angeles, Costa Mesa, Inglewood, Long Beach, Diamond Bar, Lakewood, Huntington Beach, <br />Lynwood, Riverside, Buena Park, Irvine, Santa Ana; the Los Angeles Unified School <br />District; Alhambra School District; SANBAG; Caltrans; Los Angeles Metropolitan Transit <br />Authority (MTA); and the Orange County Transportation Authority (OCTA). AESCO <br />currently holds the on -call contract for testing and inspection services for the City of <br />Santa Ana Capital Improvement Projects. <br />AESCO has NEVER had any OSHA citations. AESCO has never had any outstanding or <br />pending complaints through the Better Business Bureau, State of California Department of <br />Consumer Affairs. <br />Sub -Consultants <br />Diaz•Yourman & Associates (DYA) is a privately held geotechnical consulting services <br />corporation, founded in December 1992. DYA has grown in size, capacity, and breadth of <br />services to 23 employees, including 17 engineers, the majority of whom have advanced <br />degrees in geotechnical engineering. Our engineers are supported by a staff of technicians, <br />word processors, and drafters (ArcGIS, AutoCAD and MicroStation), and an extensive <br />technical library. Our corporate office is located at 1616 East 17th Street, Santa Ana, <br />California 92705; telephone number (714) 245-2920, fax number (714) 245-2950. <br />We have been recognized by our peers for technical excellence and innovative solutions to <br />liquefaction mitigation.
