36 <br />F. For each such employee who is covered under a spouse’s non-City sponsored <br />health plan, the City will pay the employee a cash-in-lieu payment (subject to <br />taxation as wages) each month in an amount equal to one hundred percent (100%) <br />of the monthly premium amount for the City’s lowest “employee-only” coverage, <br />if said employee waives, in writing, City-paid coverage. If an employee waives <br />City provided coverage, said employee shall provide proof of medical insurance <br />coverage in a non-City-sponsored health plan. Said waiver shall include a <br />provision warning such employee that reentry into any of the City-sponsored <br />plans is allowed only at open enrollment, unless there is a qualifying event, and <br />may require proof of insurability for such employee and/or family. <br />12.2 Dental Insurance. The City agrees to contribute towards the cafeteria Benefit plan the <br />payment of premiums for dental insurance plans provided by the City for employees and <br />their eligible dependents on the following basis: <br />A. One hundred percent (100%) of the premium cost for “employee-only” coverage. <br />B. Up to one hundred ten dollars ($110) per month per employee for “family” <br />coverage. <br />C. Effective January 1, 2023, the City will contribute up to one hundred thirty dollars <br />($130) per month per employee for “family” coverage. <br />Any contribution necessary to maintain benefits under said dental plans in excess of the <br />amount of the City contribution to the cafeteria plan specified above shall be borne <br />entirely by the employee. <br />For each such employee who is covered under a spouse’s non-City sponsored dental plan, <br />the City will pay the employee a cash-in-lieu payment (subject to taxation as wages) each <br />month in an amount equal to one-hundred percent (100%) of the City’s contribution, if <br />the employee waives, in writing, City-paid coverage. Such waiver shall include a <br />provision warning such employee that reentry into any City-sponsored plan is allowed <br />only at open enrollment, unless there is a qualifying event, and may require proof of <br />insurability for such employee and/or family. <br />12.3 Disability Insurance. The City shall continue to pay one hundred percent (100%) of the <br />premium cost for a long term disability insurance plan (with a sixty (60) day elimination <br />period) for employees. . Employees shall receive a monthly benefit of sixty-six and two- <br />thirds percent (66 2/3%) of their base monthly salary (to a maximum monthly benefit of <br />$7,500). Effective January 1, 2023, employees shall receive a monthly benefit of sixty-six <br />and two-thirds percent (66 2/3%) of their base monthly salary (to a maximum monthly <br />benefit of ten thousand dollars ($10,000)). <br />12.4 Life Insurance.
