A sample is provided for <br />reference. <br /> <br /> <br />VERIFICATION AND ELIGIBILITY <br />The City shall verify all information as necessary. All complete applications will be date stamped <br />when received and processed in that order. Applications will be subject to review by Planning and <br />Building Agency. Applications will be evaluated based on the program’s evaluation rubric. <br />Applicants will be notified in writing regarding their eligibility status.
