Grant Agreement No. 4600011889 <br />Page 28 of 32 <br />EXHIBIT F <br />REPORT FoRMATS AND REQUIREMENTS <br />The following reporting formats should be utilized. Please obtain State approval prior to submitting a <br />report in an alternative format. <br />PROGRESS REPORTS <br />Progress reports shall generally use the following format. This format may be modified as necessary to <br />effectively communicate information. For each project, discuss the following at the task level, as <br />organized in Exhibit A (Work Plan): <br />• Estimate of percent work complete. <br />• Milestones or deliverables completed during the reporting period. <br />• Discussion of work accomplished during the reporting period and submission of <br />deliverables per Exhibit A. <br />• Scheduling concerns and issues encountered that may delay completion of the task. <br />• Work anticipated for the next reporting period. <br />• Updated schedule or budget inclusive of any changes that have occurred. <br />FINAL REPORT <br />The Final Report shall generally use the following format. This format may be modified as necessary to <br />effectively communicate information on the various projects in the IRWM Program funded by this <br />Grant Agreement, and includes the following: <br />Executive Summary <br />The Executive Summary consists of a maximum of twenty (20) pages summarizing information <br />for the grant as well as the individual projects. <br />Stakeholder Summary <br />• General description of water management needs of DACs, Economically Distressed Areas <br />(EDAs), and underrepresented communities at the Funding Area learned from the activities <br />performed in this program <br />• General summary of DACs, EDAs, and underrepresented communities involved in IRWM <br />efforts through this Program <br />• Map(s) identifying all DACs, EDAs, and underrepresented communities with IRWM regions <br />learned from the activities performed in this program <br />Involvement Activity Summary <br />• General description of involvement activities performed in this Program, including both <br />successful and unsuccessful involvement activities <br />• Identification of projects developed from the DAC involvement activities, if applicable <br />Findings <br />• Needs Assessment <br />o Narrative summary of community characteristics identified and specific community <br />water management needs and resources (technical, managerial, and financial) to <br />address the needs of DACs, EDAs, and underrepresented communities <br />o Needs Assessment template table filled in (at the community level) <br />• Identification of ongoing barriers for DAC involvement in IRWM efforts <br />
