OC CHILDREN'S <br />THERAPEUTIC ARTS <br />2215 N Broadway• Santa Ana, CA 92706 • 714.547 .5468 • Fax 714.564.9690 • www.occtac.org <br />Employment for Currently Enrolled Youth: <br />To motivate youth to complete their employment goals participants will receive <br />an incentive when they meet at least one of the following goals and provide <br />supporting documentation of completion: <br />•Turn in application for a job and get an interview: $25 Gift Card incentive <br />•Obtain employment: $50 Gift Card incentive <br />•Employed for 3 consecutive months: $50 Gift Card incentive <br />Employment for Youth in Follow-Up: <br />•Turn in application for a job and get an interview: $25 Gift Card incentive <br />•Obtain employment: $50 Gift Card incentive <br />•For each quarter employed: $SO Gift Card incentive <br />Post-Secondary Education for Currently Enrolled Youth: <br />Participants are encouraged to complete their goals toward post-secondary <br />education so that they may advance in their careers and increase their chances <br />of earning higher hourly wages. Participants will be eligible for a $100 Gift Card <br />incentive if they have achieved one of the following goals toward post-secondary <br />education and provide supporting documentation of completion. <br />•Obtaining a Credential such as: Associates Degree or Certificate <br />•Registering for classes for spring or fall semester and FAFSA/Dream Act <br />Application <br />Post-Secondary Education for youth in Follow up: <br />•Obtaining a Credential: $25 Gift Card incentive <br />•Registering for Fall or Spring classes: $25 Gift Card incentive <br />Attachment A <br />3 <br />Revised: 05/07/2021 <br />EXHIBIT 1
