0RFQ For On -Call Demolition Service <br />MACRO-Z-TECHNOLOGY <br />Part 2 1 Scope of Services & Schedule <br />Proposal shall include a Scope of Services and Schedule which details the work phases to be <br />completed, the tasks to be accomplished, the deliverables to be provided, and the schedule / timeline <br />to complete the project, based upon the requested Scope of Work detailed in Attachment 1 of this RFP. <br />The scope of services and schedule outlined below is tailored specifically to the sample project, demolition of <br />1301 W. 11th Street, Santa Ana, CA. This scope includes the work phases to be completed, the tasks to be <br />accomplished in each phase, and deliverables to be provided. Following the scope of services is a graphic <br />schedule that shows the schedule / timeline to complete the project. <br />2.1 Phase 1: Request for TO Quote <br />• The City has provided a request for TO quote as part of this RFQ. <br />• MZT has reviewed the documents provided and submitted RFIs to clarify scope items. <br />• Since the project is already completed, we couldn't attend a site visit. However, we used Google Earth <br />Pro to review historical satellite imagery of the project site. <br />• Our response is included herein, and includes a fee schedule, scope of services summary, and work <br />schedule from NTP to TO completion. Note that this phase is not reflected on the attached schedule <br />because the schedule covers activities after NTP. <br />Defiverables for Phase I <br />• Fee schedule <br />• Scope of services summary <br />• Work schedule <br />2.2 Phase 2: Pre -Demolition Preparation & Mobilization <br />2.2.1 Task 2.1: Pre -Demolition Preparation <br />• Hold a preconstruction meeting with the City to confirm expectations, site contacts, and schedule <br />• Obtain a demolition permit and a plumbing permit for utility capping, if applicable <br />• Prepare & submit a demolition and grading plan (including schedule) to the City <br />• Schedule water and sewer cap inspections with the City <br />• Coordinate with utility owners (e.g., call Dig Alert 811) to identify, locate, and mark utilities <br />o For this project, we will need to schedule power, water, sewer, and gas shut off <br />o Coordinate with Southern California Edison to schedule power removal <br />o Coordinate with So Cal Gas for capping the gas line <br />o Coordinate with the water company (Santa Ana Municipal Utility Services Section) to schedule <br />capping for the water and sewer lines <br />• Concurrent with utility coordination, submit the AQMD notification (14-day notice, if applicable) <br />• Hand out City -provided demolition notices to the project's neighbors <br />Delliverables for Task 2.1 <br />• Preconstruction meeting agenda and minutes <br />• Demolition permit <br />• Demolition and grading plan <br />2.2.2 Task 2.2: Mobilization <br />• Reach out to subcontractors to line up abatement support <br />• Allocate internal resources (equipment, manpower, tools, materials, supplies) <br />o Specific equipment and materials are mobilized to the site when needed <br />o This keeps the site more organized, which is better for safety <br />o See Table 2 for the equipment, crew, tools, and materials / supplies we anticipate using for the <br />sample project <br />• Erect site fence <br />• Install SWPPP / BMPs (straw wattle, silt fence, rumble plate(s)) <br />
