STATEMENT OF QUALIFICATIONSCONTENTS <br />Statements of qualifications are to be prepared in such a way as to provide a <br />straightforward, concise delineation of capabilities to satisfy the requirements of this RFQ. <br />Colored displays, promotional materials, etc., are not necessary or desired. Emphasis <br />should be concentrated on conformance to RFQ instructions, responsiveness to the RFQ <br />requirements, and on completeness and clarity of content. Dividers and clear organization <br />of content and material are encouraged. <br /> <br />1. Cover Letter <br />Statements of qualifications shall include a letter signed by a principal or authorized <br />representative who can make legally binding commitments for the entity. Include type <br />of business entity. Cover Letter shall not exceed one page. <br />Cover letter must be addressed to the following: <br />Daniel A. Soto, Management Assistant <br />City of Santa Ana – City Manager’s Office <br />20 Civic Center Plaza <br />Santa Ana, CA 92701 <br /> <br />2. Agreement Statement <br />Statement of qualifications shall include a statement outlining your concurrence or <br />concerns with any and all provisions as contained in the Standard Agreement attached <br />as as Exhibit A of this RFQ. <br />3. Firm and Team Experience <br />Statement of qualifications shall include a profile of the firm’s experience including the <br />following: <br />i. A general description of the firm, including size, number of employees, number <br />of credentialed actuaries with their affiliations, and any past or contemplated <br />changes in ownership. <br />ii. Name and contact information of the lead project manager to be assigned to <br />the engagement. <br />iii. Resumes for the professional staff assigned to the engagement. <br />iv. A description of the firm's experience in providing similar services to those <br />requested in this RFQ. <br />v. A list of the local office's most significant engagements in the last five (5) years, <br />indicating whether they are public or private sector, and including scope of <br />work, date, supervising actuaries, and name and telephone number of the <br />client contact. <br />
