TABLE OF CONTENT <br />The format of the table of content should be straight forward, with each section followed by a <br />page number. <br />3. EXECUTIVE SUMMARY <br />This section should be limited to a brief narrative highlighting the Company's proposal. The <br />summary should be free of technical language and should illustrate the benefits and <br />possibilities offered by the Proposer. It should be between one (1) and five (5) pages long. <br />There should be no mention of the projected revenue in this section. <br />4. COMPANY AND TEAM EXPERIENCE <br />Proposal shall include a profile of the company's experience. Resumes of project team/sub- <br />contractors that outline their technical and design experience should be included. At a <br />minimum, this should include the project manager/principal agent, associates in charge when <br />project manager/principal agent is unavailable, key personnel, firm size, and an organization <br />chart identifying only those who will perform work for the proposed project and the percentage <br />of each individual's time devoted to this project. The project manager/principal agent shall be <br />the primary contact person to represent the company. <br />Proposers must provide the following information about their company that demonstrates its <br />stability and ability to support the commitments set forth in the RFP. Proposers should outline <br />the company's background, including: <br />a. How long the company has been in business. <br />b. A brief description of the company size and organization. <br />c. Number of customers in California. <br />d. Total number of customers. <br />e. Company organizational chart. <br />f. Financial data, including the most recent audited financial statements (MARKED <br />"CONFIDENTIAL"). <br />5. STATEMENT OF QUALIFICATIONS <br />The Statement of Qualifications shall be limited to a MAXIMUM of (10) DOUBLE -SIDED <br />PAGES (excluding front and back covers, section dividers and attachments such as resumes, <br />forms). <br />6. COMPANY IDENTIFICATION AND EXPERIENCE <br />a. The name, address, and telephone numbers of the Company, along with resumes and a <br />description of background experience of each person with whom the City must <br />communicate. <br />Page 12 of 20 <br />
