Therefore, do not include any <br />information that is subject to state or federal confidentiality regulations (i.e. an address to a <br />domestic violence shelter). <br /> <br />The attached application contains several questions regarding city residents. Residency <br />of participants is determined by the last permanent address. The City will hold <br />organizations accountable for the information included, such as expected service level. <br /> <br /> <br />Review of Applications <br />City staff will conduct a minimum threshold review of all applications received for completeness <br />and to determine the eligibility of the proposed program. The Community Development <br />Commission (CDC) will review and recommend public service funding levels to City Council.
