• Review of School Catalog (rules and regulations of the school and student rights) <br />Program Specific Admissions Requirements: <br />Applicants for Phlebotomy Technician I must have: <br />Proof of High School graduation is required for this program. A standard high school diploma or General Education <br />Development (GED) certificate Is required. Additionally, when applying for state licensure with the Department of Health <br />Services, students must have their sealed official high school transcripts or GED certificate sent directly the Department of <br />Health Services. As part of the licensure application process, the Department of Health Services requires foreign transcripts <br />to be evaluated by a current members of the National Association of Credential Evaluation Services (NACES) or endorsed <br />members of the Association of International Credential Evaluators, Inc. (AICE). Phlebotomy Technician I students will also be <br />subject to a background check by the Department of Health Services before as part of the licensure eligibility determination. <br />Applicants for admission into the Clinical Medical Assistant Program must have: <br />1. A standard High School Diploma or General Education Development (GED) certificate with proof of graduation. An applicant <br />who Is not a high school graduate or has not passed the GED may be admitted upon successfully passing an Entrance <br />Examination. <br />Applicants for admission Into the Emergency Medical Technician Program must have: <br />1. A standard High School Diploma or General Education Development (GED) certificate with proof of graduation. <br />2. The student must at all times maintain a current Basic Life Support for Healthcare Provider certification from the American <br />Heart Association, or a Professional Rescuer certificate from the American Red Cross. No other certifying or training entity <br />nor their curriculum will be accepted. <br />Applicants for admission into the Nurse Assistant Training Program must have: <br />1. A standard High School Diploma or General Education Development (GED) certificate with proof of graduation. An applicant <br />who is not a high school graduate or has not passed the GED may be admitted upon successfully passing an Entrance <br />Examination. <br />2. Must pass a criminal background check on or before the first day of class via Live Scan. <br />3. A medical history and physical examination signed by the examiner Indicating that the student does not have any health <br />condition that would create a hazard to themselves, fellow employees, or patients and must be done within 90 days prior to <br />the start date of the program. <br />4. A test for tuberculosis Infection that Is recommended by the federal Centers for Disease Control and Prevention (CDC) and <br />licensed by the federal Food and Drug Administration (FDA), unless medically contraindicated. If a positive test result is <br />obtained, a chest x-ray shall be taken, unless medically contraindicated. This test must be done within 90 days prior to the <br />start date of the program. - <br />ADMISSION OF STUDENTS WITHOUT HIGH SCHOOL DIPLOMA <br />Applicants without a standard High School Diploma or General Education Development (GED) certificate with proof of <br />graduation may apply for admission Into the following programs only: Clinical Medical Assistant, Nurse Assistant and <br />Training Program. <br />Applicants must complete an interview with a school representative to determine desire, ability and commitment, receive a tour <br />of the campus and review the school catalog. Once an applicant has passed an entrance examination, the applicant may then <br />complete and sign an Enrollment Agreement. <br />An applicant who does not have a standard High School Diploma or General Education Development (GED) certificate, must <br />take and pass the Wonderlic Scholastic Level exam to measure the applicant's cognitive ability with a minimum score as listed <br />by program. <br />clinical Medical Assistant -14 minimum score <br />Nurse Assistant and Training Program - 13 minimum score <br />Students who do not achieve a minimum score for the program for which they are applying, may retake the exam one time. <br />ADMISSION PROCEDURE <br />The admission procedure starts with the exchange of Information between the applicant and the school. Our Admissions <br />representatives Interview applicants on campus. During this Interview, an admissions representative evaluates an applicant's <br />history, careergoals, and potential for academic success. If the applicant has special needs and circumstances, It Is discussed at <br />this time to ensure that the school can provide what Is needed to enable the student to successfully complete their program prior <br />to enrolling in the school. Tuition and payment options are also presented in writing and discussed prior to enrollment. <br />ACCEPTANCE OF CREDITS EARNED AT OTHER INSTITUTIONS <br />Universal Schools & Colleges of Health & Human Services does not accept credits earned at other Institution or through challenge <br />examinations or achievement tests. <br />TRANSFER OR ARTICULATION AGREEMENTS <br />This Institution has not entered Into a transferor articulation agreement with any other college or university. <br />
