NOTICE CONCERNING TRANSFERABILITY OF CREDITS AND CREDENTIALS EARNED AT OUR INSTITUTION <br />The transferability of clock hours you earn at Universal Schools & Colleges of Health & Human Services is at the complete discretion <br />of an Institution to which you may seek to transfer. Acceptance of the certificate you earn in the educational program is also at <br />the complete discretion of the Institution to which you may seek to transfer. If the certificate that you earn at this institution is not <br />accepted at the Institution to which you seek to transfer, you may be required to repeat some or all of your coursework at that <br />Institution, For this reason, you should make certain that your attendance at this Institution will meet your educational goals. This <br />may include contacting an Institution to which you may seek to transfer after attending Universal Schools & Colleges of Health & <br />Human Services to determine if your certificate will transfer. <br />FINANCIAL ASSISTANCE <br />Universal Schools & Colleges of Health & Human Services offers lending options through an Independent student loan company <br />for all programs. In order to qualify for a student loan, the student must apply with the student loan company, additional <br />information will be provided by your admissions advisor. If a student defaults on their loan, they will be considered as non - <br />graduates of the program In which they enrolled. <br />Students who finance their program must keep their account current by making on -time payments. Any late/non-payment of a <br />loan may result In a negative report to the student's credit bureaus and may also result in not being allowed to progress through <br />the program and/or termination from the program. <br />If a student obtains a loan to pay for an educational program, the student will have the responsibility to repay the full <br />amount of the loan plus interest, less the amount of any refund, <br />Continuing Education Scholarship <br />Current Phlebotomy students that have completed the didactic portion of their program and are waiting to start their externship <br />may be eligible for 20% off tuition for any subsequent program that he/she enrolls In at US Colleges. To be eligible, students must <br />be in good academic standing in their current program and be current on their tuition payments. This scholarship will be credited <br />to the student's account upon starting and attending classes and cannot be combined with any other tuition credits or scholarship <br />programs. There Is no form or application to complete in order to participate in this scholarship program. <br />Graduate Scholarship <br />Graduates from any US Colleges or LEI College program may be eligible for a 20% off the tuition price for any subsequent <br />programs that he/she enrolls in at US Colleges. This scholarship will be credited to the student's account upon starting and <br />attending classes and cannot be combined with any other tuition credits or scholarship programs. There is no form or application <br />to complete in order to participate in this scholarship program. <br />Work Innovation and Opportunity Act <br />WIOA or the Federally Funded Work Innovation and Opportunity Act provides students the chance to be successful in the Job <br />market by providing them with access to training, employment, and support services while working to match them with employers <br />based on their skills and training. <br />HOLIDAYS <br />Nine (9) official holidays are observed during the calendar year. They are as follows: <br />New Year's Day Martin Luther King Jr. Day <br />Spring Holiday Memorial Day <br />Independence Day Labor Day <br />Thanksgiving Day, Friday After Christmas Eve <br />Christmas Day <br />The school closes for classes for one to two weeks between Christmas and New Year's depending on where the holidays fall in <br />relation to the school schedule. <br />ATTENDANCE POLICY <br />All Clinical Medical Assistant students must attend 80 percent of their schedule. Attendance Is monitored daily, recorded In the <br />student's flies, and reported every two weeks in the progress report. All Phlebotomy and Emergency Medical Technician and <br />Nurse Assistant Training Program students must complete 100%of their hours for program completion. <br />Progress of attendance Is monitored, and the cumulative number of absences are counted each quarter of the program and <br />recorded on the student's progress report, <br />Students who have been absent for three consecutive days must receive advising from a member of Administration or their <br />Instructor before returning to his/her classes. Students who have unexcused absences for five consecutive school days may be <br />terminated. This determination will be made by the School Director. Students that fail to attend class for 14 calendar days may be <br />considered as "withdrawn from the program". <br />10 <br />
