Assume 20 hours of witness testing/on-site services per site <br />4. Source Control <br />a. Fee for initial PLC program collection <br />b. Fee for establishing functioning Source Control database, standards and <br />procedures <br />c. Annual budget for processing check-in/check-out requests <br />i. Assume 4 requests per month <br />5. Change Control <br />a. Fee for establishing functioning Change Control forms, standards and procedures <br />b. Annual budget for processing/documenting change control requests <br />i. Assume 4 requests per month <br />6. Firm's standard hourly fee schedules <br />Furthermore, the Consultant shall submit additional labor, material and rental equipment <br />rates along with fee schedule. Consultant's labor and equipment rate sheet shall list rates for <br />all labor designations, equipment, rentals, and materials. The bid items specified in the fee <br />schedule are for reference purposes only. Labor increases shall be subject to mutually agreeable <br />terms between the City and the Consultant. The City may request related services that will be <br />paid at the vendor's standard labor and equipment rate submitted.
