Consolidated Annual Performance and Evaluation Report for Fiscal Year 2022-23 <br />September 19, 2023 <br />Page 3 <br />• Community Health Initiative of OC and Radiant Health <br />Services provided access to health and social service <br />programs by conducting outreach, education, and <br />enrollment. <br />• OC Fair Housing Council and Public Law Center <br />provided housing education, homelessness services, <br />and counseling. <br />• Community Action Partnership of Orange County <br />provided tools to increase financial well-being and <br />improve stability. <br />• The Westview House affordable housing project located <br />at 2530 & 2534 Westminster Avenue, Santa Ana, CA <br />92701 continued construction. This project will provide <br />84 units of affordable housing for large families including <br />nine (9) HOME assisted units. <br />• The Crossroads at Washington affordable housing <br />project located at 1126, 1136, and 1146 E. Washington <br />Avenue continued construction. This project will provide <br />85 units of affordable housing for large families including <br />12 HOME assisted units. <br />• The WISEPlace Permanent Supportive Housing project <br />Affordable Housing <br />located at 1411 N. Broadway closed on their financing <br />and began construction. This project will provide 47 <br />units of permanent supportive housing including 16 <br />HOME -ARP assisted units. <br />• Funded nine (9) first-time homebuyer down payment <br />assistance loans with $1,080,000 in CDBG funds. <br />• Participated in eight (8) down payment assistance <br />workshops and three (3) community fairs. <br />• Completed a total of 10 residential rehabilitation projects <br />for six (6) very -low to extremely -low income residents <br />and four (4) low income residents. Rehabilitation work <br />included the complete repair of roofs, ADA <br />improvements. and plumbing upgrades to restrooms. <br />• Community preservation efforts continue in deteriorated <br />and deteriorating areas to preserve the City's aging <br />housing stock. In FY 2022-23 Code Enforcement funds <br />were used to respond to complaints covering residential <br />properties within the CDBG eligible deteriorating and <br />Code Enforcement <br />deteriorated areas. During the program year, the City <br />issued a total of 6,336 violations to single-family units <br />and 100% of the violations were corrected by <br />homeowners. <br />• Code Enforcement staff also issued 209 Notices of <br />Violation, 195 Notice and Orders, and 11,133 <br />
