CITY OF SANTA ANA <br /> <br /> <br /> <br /> <br /> <br />1. Firm: A general description of the firm, including size and number of employees <br />working directly with the City on this agreement. Firm’s nearest address serving <br />the City of Santa Ana and headquarters address. <br />2. Project Manager/Principal Agent: Name and contact information of the <br />supervising Project Manager/Principal Agent, to be assigned to the agreement. <br />The Project Manager/Principal Agent shall be the primary contact person to <br />represent your firm and will be the person to conduct the presentation if invited to <br />an interview. <br />3. Vendor(s) Team: An organizational chart showing lines of responsibility, as well <br />as a list of team members and their duties as part of the team. If the Vendor(s) is <br />a nonprofit corporation, or if a nonprofit is one of the team members, the SOQ <br />must include documentation that the corporation is certified by the U.S. Internal <br />Revenue Service as a 501(c)3 tax-exempt non-profit corporation, and is in good <br />standing with both state and federal compliance. <br /> <br />4. Vendor(s) Experience: A narrative describing your firm’s demonstrated <br />capability, including the length of time that your firm has provided the services <br />being requested in this RFP. Some members of staff shall be bilingual. <br />Experience gained in the last five years on projects similar to the one being <br />proposed. <br /> <br />i. List two recent similar projects delivered by the firm for other public agencies or <br />community-based organizations including the criteria listed below: <br /> <br /> Include a description of the summary of work performed <br /> The project name and type (special needs, senior, large family, etc.) <br /> The total project cost. <br /> The funding sources. <br /> The percentage of work the proposer was responsible for. <br /> The period over which the work was completed, <br /> The name, title, and phone number of clients to be contacted for references. <br /> <br />ii. A Letter of Support from a community member, community-based organization, or <br />public agency may serve as a substitute for one of the two projects. <br /> <br />iii. Include a description of your experience doing community outreach and capacity <br />including resources, staffing, and available funding. <br /> <br />1. Financial Capacity: Description of the Vendor’s financial strength and ability to <br />obtain project financing, and to provide sufficient equity for the successful completion <br />of the proposed project. <br /> <br />i. Include a description of current relationships with major lending institutions. <br /> <br />ii. Any Vendor(s) that is selected for recommendation to City Council, or if a <br />partnership, the team members who will retain an ownership interest in the <br />project, will be required to submit complete financial statements for the last <br />three years.
