The Peer case manager/facilitator will be responsible for conducting <br />outreach at local schools, organizations, venues and places in Santa Ana where young TGNC <br />individuals congregate. The group facilitator will also serve as a mentor to the support group <br />participants and will be able to connect or refer clients to other supportive services within ATL or <br />out in the community as needed. Together and with the support of the administrative specialist <br />the group facilitator/peer case worker will work on reporting back to the city of Santa Ana <br />regarding the proposed program. <br />We are very much aligned with both Peer mentorship and Re-entry principles in that leadership <br />and staff agree that: <br />• Connecting Trans youth to supportive services is a first step to improving their quality of <br />life and is the most humane approach to service provision. <br />• Supportive services are offered to maximize personal stability, not as a precondition to <br />be met to attain help. <br />• The criminal legal system is a part of our world, and we should work to minimize its <br />harmful effects rather than simply ignore or condemn them. <br />• Services for TGNC people who have been through the carceral system should be non- <br />judgmental and non -coercive. <br />Estimated Timeline and description of activities proposed for the pilot program: <br />During the first year (03/01/2024-02/28/2025) of the TGNC Youth Support group program, we <br />expect to be able to Conduct at least 48 support groups (4 per month X 12 months = 48). Recruit <br />at least 10 TGNC youth participants that reside in Santa Ana to provide them with group support <br />services and peer mentorship. Additionally, we expect to conduct at least two small events <br />specifically designed for TGNC youth. <br />1. 03/01/2024 we will Recruit, hire and train a support group facilitator: <br />a. Create and distribute job descriptions <br />b. Interview candidates and hire <br />c. Train new staff in program goals and agency protocols. <br />d. Group facilitator will develop an outreach calendar to recruit support group <br />participants for the 2023-2024 cohort. <br />2. From 03/01/2024 to 02/28/2025 we will begin providing services through weekly support <br />groups; educational resources/workshops, health resources/workshops, mentoring, <br />counseling, and other life skills or workforce skills: <br />a. Develop and distribute outreach materials; network and participate in outreach <br />events. <br />b. Meet with clients in a group setting and conduct a strength -based assessment to <br />further understand participants' needs. <br />c. Group facilitator works with support group clients to identify important support <br />group topics <br />d. Develop and design group comment cards and distribute to obtain participant's <br />feedback <br />f1 <br />
