Statement of Qualifications <br />Services Provided: <br />The services provided by HEART meet the needs of the City as described in section III as HEART <br />will be able to provide spaying and neutering services for dogs and cats via a mobile clinic. <br />Agreement Statement: <br />Any and all staff included in these mobile clinics whose positions require a license or <br />certification of any kind, shall have the necessary requirements. The following are all key <br />professional staff members and their titles: <br />1) Todd Kopit is the chief veterinarian for HEART and is a doctor of veterinary medicine <br />licensed in the state of California. <br />2) Elizabeth Edmunson is a doctor of veterinary medicine licensed in the state of California. <br />3) Yikcia Milanes-Pope is a doctor of veterinary medicine licensed in the state of California. <br />4) Carolyn Ruff is a registered veterinary technician in the state of California. <br />5) Benjamin Shiffman is a registered veterinary technician in the state of California. <br />6) Joshua Brown is a registered veterinary technician in the state of California. <br />Firm and Team Experience: <br />i) HEART is a 501(C)(3) organization that provides low-cost veterinary services, focusing <br />on serving the needs of pet owners in underserved communities. HEART employs 15 <br />staff members in positions ranging from administrative to medical staff and part-time <br />to full-time. At the type of clinic requested in the agreement by the City of Santa <br />Ana, an average of 6 staff members will be needed. <br />ii) HEART is strictly a mobile organization and has previously served the City of Santa <br />Ana by providing vaccine, wellness, and spay/neuter clinics within the city, <br />surrounding cities, and other municipalities in several counties. HEART's <br />headquarters are located at 4311 Aspenglow Circle Yorba Linda, CA 92886. <br />iii) The Project Manager assigned to this agreement shall be Deborah Kopit, Operations <br />Manager. <br />iv) Resumes are included in Attachment G. <br />Proposed Work Plan: <br />HEART shall conduct 1-2 monthly spay and neuter clinics for residents within the allowable <br />limits at a location within the City of Santa Ana that has been pre -approved by the City's Parks, <br />Recreation, and Community Services Agency. If a location cannot be reserved for these services, <br />HEART will perform the clinics at the Orange County Animal Care parking lot in the neighboring <br />city of Tustin or at a suitable location in other surrounding cities such as a privately owned <br />
