<br /> Annual Action Plan <br />2024 <br />53 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />AP-65 Homeless and Other Special Needs Activities – 91.220(i) <br />Introduction <br />The needs of individuals experiencing homelessness and persons with special needs are complex and <br />require a wide range of specialized services. Numerous agencies are typically involved in the care of <br />these individuals, providing distinct services such as housing, mental health counseling, employment <br />training, and case management services. A number of activities and services are funded to help the <br />needs of individuals experiencing homelessness and other special needs populations. Overall, these <br />services address the high priority of reducing homelessness and the threat of becoming homeless, as <br />well as providing necessary supportive services. <br /> <br /> <br />Describe the jurisdictions one-year goals and actions for reducing and ending homelessness <br />including <br />Reaching out to homeless persons (especially unsheltered persons) and assessing their <br />individual needs <br />Through this Action Plan, the City of Santa Ana will continue to prioritize support and assistance to <br />unhoused individuals and those at imminent risk of becoming homeless. Through HUD and non-HUD <br />resources, the City will continue to: <br />1. Prioritize street outreach through the Quality of Life Teams/HEART and contracted outreach and <br />engagement teams. <br />2. Preserve existing and increase the supply of permanent supportive housing <br />3. Preserve existing and increase the supply of affordable housing <br />4. Provide housing services and assistance to special needs populations <br />5. Improve critical services to low-income and special needs populations <br />6. Coordinate services within the City as well as regionally in collaboration with the Continuum of <br />Care <br />7. Collaborate with all communities in Orange County to address homelessness with coordinated, <br />regional approaches <br />8. Operate a low-barrier, year-round, 24-hour, 200 bed Navigation Center. <br />9. Invest in homelessness diversion and prevention strategies and programming. <br />10. Develop a homelessness response system that reduces the growing number of <br />individuals & families, who identify as BIPOC, and are experiencing homelessness. <br />11. Fund programming and solutions that assist individuals and families experiencing <br />homelessness become permanently and stably housed, including RRH programs <br />for individuals and TAY. <br />EXHIBIT 1
