<br /> Annual Action Plan <br />2024 <br />55 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />management services are offered in all programs to help prevent individuals from falling back into <br />homelessness. ESG Rapid Re-Housing dollars are utilized to in this effort. The Santa Ana Housing <br />Authority along with other Orange County Housing Authorities recently entered into an agreement with <br />Cal Optima to address the complex challenges of people facing housing insecurity, offering care <br />coordination and a broad range of community-based services to improve health outcomes. <br />Helping low-income individuals and families avoid becoming homeless, especially extremely <br />low-income individuals and families and those who are: being discharged from publicly <br />funded institutions and systems of care (such as health care facilities, mental health facilities, <br />foster care and other youth facilities, and corrections programs and institutions); or, receiving <br />assistance from public or private agencies that address housin g, health, social services, <br />employment, education, or youth needs. <br />The ESG program will fund homelessness prevention and rapid re-housing programs including utility and <br />security deposit assistance as well as rental assistance. The City’s Outreach teams provide bus tickets for <br />individuals seeking family reunification; often following a release from a correctional facility. These <br />teams will also continue to partner with local hospitals to provide trainings to address discharge. The Jail <br />Release Outreach program will continue to offer resources for individuals exiting the Santa Ana <br />Jail. Additionally, the County of Orange is equipped to serve people discharged from publicly funded <br />institutions or systems of care such as health care facilities or correction programs. The County and City <br />Navigation Centers are options for individuals who have no other permanent housing option. <br /> <br /> <br />Discussion <br />n/a <br /> <br />EXHIBIT 1
