is trained on using our Hope Inaex Assessment to measure aenats in our stuaents socto-emotional aeveiopment <br />across 10 domains, and monitoring the levels of response in order to adapt the goal plan that guides each <br />student's progress through our program. <br />Our goal is to create an environment of equity, so that these youth have equal access to the same opportunities as <br />their housed peers, which also includes the ability to participate in cultural and community events. Samples of <br />current enrichment activities include college visits (for high schoolers), movie and beach days, summer and sports <br />camps, book clubs, skills -based workshops, holiday events, birthday celebrations, and museum and aquarium <br />visits. <br />Our Santa Ana team of Case Managers are 100% Hispanic and bilingual, reflecting the demographics of the <br />students we serve in the district. And more importantly —they are well -versed in the myriad ways we help the <br />students overcome the cultural barriers confronting Hispanic students who are experiencing homelessness. Our <br />solution is not prescriptive, but tailored to fit the specific needs of each individual child no matter their gender, <br />race or ethnicity. As such, we pull from a "toolbox" of resources than can include any/all of the following: <br />- Individualized academic support & materials <br />- Weekly mentoring and tutoring (based on availability of tutors) <br />-Teacher coordination <br />- Parent involvement <br />- Personalized one-on-one Case management <br />- Definition of goals and a case plan <br />- Home visits/weekly check -ins <br />- McKinney Vento Coordination <br />- 24/7 support <br />- Connections to needed community resources, such as housing, food and healthcare <br />- Career coaching, definition of future academic goals <br />- Collaboration with teachers, counselors, and administrators <br />- Assistance with college applications, and financial assistance (like FAFSA) <br />- Access to enrichment activities, such as those mentioned above <br />- 24-hour access to our case managers, 7 days a week <br />Outcomes: <br />1.Case Management: Provide holistic supportive services, centered around an intensive case management model, <br />to a minimum of 60 Santa Ana residents, adolescents and young adults (up to age 24) who are experiencing <br />barriers to education and employment due to homelessness. <br />2. High School Graduation: Our Project Hope Alliance high school seniors will have an on -time graduation rate of <br />80% or higher. Graduation rates are also tracked in our Salesforce database. In June 2023, 100% (6 out of 6) of our <br />SAHS seniors graduated (while 93%- a total of 24 - of our seniors across all Orange County graduated). This is <br />contrasted with the average graduation rate for homeless youth in California of 69.7%. <br />3. Group Work: This past year we have revised our Group Work facilitation of relevant workshops for youth. The <br />goal of Group Work is to provide youth with relevant skills in a manner that builds upon previous skills. We also <br />use these topics to help guide our case planning. This year, we implemented Group Work in the following topics: <br />Goal Setting, Stress Management, Healthy Relationships, and Online Safety. We also are embedding regular <br />workshops for youth utilizing a "roadmap" to guide students to a college education, coaching on FAFSA/ Financial <br />Aid and how to fund a college experience, and Employment Readiness. We project 9 of our Santa youth (under <br />this funding) will attend Group Work sessions in the new school year. Overall, across all districts, we project <br />serving 125 students through the Group Work program. <br />4. Post-HS Pilot Programming: Project Hope Alliance will launch a college and career readiness pilot program for 4 <br />transitional age youth (of the 60 youth under this funding opportunity) that encompasses workshops focused on <br />life skills, navigational skills, postsecondary readiness, and career exploration. <br />Budget: <br />$44,647 per year for the support of four Case Managers dedicated to Santa Ana youth, who qualify as homeless <br />under the McKinney-Vento Act of 1987. 35 students will be served. <br />over a two-year term <br />EXHIBIT A <br />
