In <br />early intervention, child development specialists identify and treat cognitive, social, and motor skill delays <br />in infants and toddlers between age zero (straight out of the NICU) to three. Pediatric therapy serves <br />children and youth between the ages of three and 22, offering multi -disciplinary support. Physical therapy <br />enhances balance, coordination, and strength while minimizing associated pain. Occupational therapy <br />fine-tunes motor skills, including handwriting and sensory processing. Speech therapy concentrates on <br />areas related to speech, fluency, and oral motor skills, fostering improved communicative abilities. Our <br />early intervention and pediatric therapy services are provided at our 18,607 square foot Pediatric Therapy <br />Center in Santa Ana, making accessing multiple services easier for children and families with disabilities. <br />Our Safety Net Fund covers the funding gap associated with providing these clinical services to <br />uninsured and under -insured children with disabilities. All children and families receiving our early <br />intervention and pediatric therapy services also have access to our family support services, which provide <br />educational workshops and support groups for families as they navigate the joys and challenges of caring <br />for a family member with a disability. <br />UP anticipates achieving the following results by the end of the 12-month grant period: <br />1) Service Delivery: <br />UP aims to provide critical early intervention and physical, occupational, and speech therapy sessions to <br />212 uninsured and under-insrued children with disabilities residing in Santa Ana during the grant period. <br />This metric will serve as an indicator of our ability to reach and serve the target population of children with <br />disabilities in need of specialized care. <br />2) Achievement of Treatment Goals: <br />We anticipate that 70% of the children we treat will achieve one or more of their treatment goals outlined <br />in their "Plan of Care" by the end of the grant period. <br />1\ Parental Pernentinn of Chilri'c Inrienenrtcnne and PrenareHnaQc fnr r.hilri'c Cara - <br />EXHIBIT A <br />
