ORDINANCE NO. NS-1394 <br />PAGE ELEVEN <br /> <br />(1) The name, address and age of the applicant, <br />and if the applicant has ever been known by <br />any other names, such other names shall be <br />set forth; <br /> <br />(2) Complexion, color of eyes and hair; <br /> <br />(3) Whether he has ever been convicted or pleaded <br />guilty or nolo contendere to any crime, felony or <br />misdemeanor, and, if so, the date, nature of the <br />offense and the court in which such charge was <br />made, conviction was obtained or plea of guilty <br />or nolo contendere was entered; <br /> <br />(4) The kind and number of his California driver's <br />or chauffeur's license and restrictions thereon, <br />if any, and whether any license to drive has ever <br />been suspended or refused or revoked, and if so, <br />details of the reason and disposition of the matter; <br /> <br />(5) A statement showing where he has resided <br />during the preceding year, together with the <br />number of days, weeks, or months at each address; <br /> <br />(6) A statement of his physical condition, in- <br />cluding a statement of whether an applicant has <br />ever had epilepsy, blackout periods, fainting <br />spells, or been addicted to the use of alcohol, <br />narcotics, or other form of dangerous drugs. <br /> <br />(7) Whether he has had any driver's license <br />suspended or revoked and, if so, the reason for <br />such suspension or revocation and the inclusive <br />dates of such suspension or revocation. <br /> <br />(b) No driver's permit shall be issued to any <br />person: <br /> <br />(1) Who is under the age of eighteen (18); <br /> <br />(2) <br /> <br />Who is subject to epilepsy, fainting or <br />loss of consciousness by reason of chronic <br />medical condition; <br /> <br />(3) Who is addicted to the use of narcotics; <br /> <br />(4) Who is known to have been a user of dangerous <br /> drugs or alcohol to an excessive degree; <br /> <br />(5) <br /> <br />Whose driving record indicates five (5) <br />violations of traffic laws involving moving <br />vehicles within two (2) years preceding the <br />date of application; <br /> <br />(6) <br /> <br />Who has willfully furnished false information <br />on his application or omitted to furnish <br />all information requested on said appli- <br />cation forms; <br /> <br />(7) <br /> <br />Who has been convicted for driving while <br />under the influence of intoxicating liquor <br />or narcotic drugs, or both, within three (3) <br />years of the date of application; or <br /> <br />(8) <br /> <br />Who has been convicted of a felony or any <br />crime involving moral turpitude within five <br />(5) years preceding the date of application. <br /> <br /> <br />
