NS- 2041 <br />PAGE SEVEN <br /> <br />(13) A description of whether the service pro- <br />posed by the applicant will include basic life <br />support services or advanced life support service, <br />and, if so: <br /> <br /> (i) The number of basic life support or <br />advanced life support service units to be deployed <br />on each shift; and <br /> <br /> (ii) The provisions, if any, for continuing <br />education of attendants. <br /> <br />(14) Such other information as the Licensing <br />officer may require in regulations adopted pursuant <br />to this Article. <br /> <br /> (c) Renewal applications shall be submitted in the same <br />form and require the same materials, as original applications <br />except the requirement of item (11) of subsection (b) of this <br />section. <br /> <br /> (d) The fees referenced in subsection (a) of this sec- <br />tion shall be the same as those charged by the County for <br />the same activities undertaken by the County with regard to <br />the licensing of ambulance services in the unincorporated <br />areas of the County, except that at any time that a County <br />City Agreement is not in effect, the City Council may, by <br />resolution, establish fees differing from the County fees. <br /> <br />Sec. 14-106. Investigations. <br /> <br /> Upon receipt of a completed application and the required <br />fee, if any, the Licensing Officer shall make, or cause to <br />be made, such investigation as the Licensing Officer deems <br />necessary to determine if: <br /> <br /> (a) The applicant is a responsible and proper person <br />to conduct, operate or engage in the provision of ambulance <br />services; <br /> <br /> (b) The applicant meets the requirements of this <br />Article and of other applicable laws, ordinances or regu- <br />lations. <br /> <br /> <br />
