L` 2.2 In order to minimize transpovendor's facility s abated within a 5 mile driving distance of the shelter. 2.3 The vendor shall, prior to contract award, shall furnish the City with an insurance certificate evidencing that it carries the amounts required as prescribed by the City Attorney and shall list the City, its officers, employees, agents, and representatives as additional insured. 3.0 PROPOSAL REQUIREMENTS 3.1 One (1) original and three (3) copies of the proposals are due in the office of the City of Santa Ana Purchasing Division, 20 Civic Center Plaza, Fourth Floor, not later than 5:00 pm, Monday, October 29, 2001. Proposals shall include the following: 3.1.1 A summary of personnel currently employed by the contractor who would be performing work under the resulting contract indicating position titles, names, years of experience, qualifications (include bilingual capabilities and specify languages), California veterinary license numbers, the number of employees in each position. 3.1.2 Provide a complete list of available services and vaccines. 3.1.3 The name, address, contact name, and phone number of five (5) clients to whom you currently provide these services. 4.0 CRITERIA FOR SELECTION 4.1 A committee of City staff members including representatives of the Finance and Police Departments and the City Attorney and City Manager's Office will evaluate proposals. The selection committee may interview final candidates. 4.2 Evaluation of proposals and interviews will include the following criteria (in no particular order): 4.2.1 Experience and reputation of the vendor as documented by reference check. 4.2.2 Experience and credentials of key personnel assigned to the project.
