• 0 <br />2.0 QUALIFICATIONS <br />2.1 ' Vendor shall possess or employ Rersons possessing a valid State of California <br />Veterinary License and valid DEA License for narcotic drugs. Vendo shall comply with all <br />requirements stated in th Californa e�rp-Pra <br />isti e Act o 9. L` <br />2.2 In order to minimize transpovendor's facility s abated within a <br />5 mile driving distance of the shelter. <br />2.3 The vendor shall, prior to contract award, shall furnish the City with an insurance <br />certificate evidencing that it carries the amounts required as prescribed by the City Attorney <br />and shall list the City, its officers, employees, agents, and representatives as additional <br />insured. <br />3.0 PROPOSAL REQUIREMENTS <br />3.1 One (1) original and three (3) copies of the proposals are due in the office of the City <br />of Santa Ana Purchasing Division, 20 Civic Center Plaza, Fourth Floor, not later than 5:00 <br />pm, Monday, October 29, 2001. Proposals shall include the following: <br />3.1.1 A summary of personnel currently employed by the contractor who would be <br />performing work under the resulting contract indicating position titles, names, years of <br />experience, qualifications (include bilingual capabilities and specify languages), California <br />veterinary license numbers, the number of employees in each position. <br />3.1.2 Provide a complete list of available services and vaccines. <br />3.1.3 The name, address, contact name, and phone number of five (5) clients to whom you <br />currently provide these services. <br />4.0 CRITERIA FOR SELECTION <br />4.1 A committee of City staff members including representatives of the Finance and <br />Police Departments and the City Attorney and City Manager's Office will evaluate proposals. <br />The selection committee may interview final candidates. <br />4.2 Evaluation of proposals and interviews will include the following criteria (in no <br />particular order): <br />4.2.1 Experience and reputation of the vendor as documented by reference check. <br />4.2.2 Experience and credentials of key personnel assigned to the project. <br />
