The AA page must <br />include a valid signature on file with Cal OES and the date. <br />Subrecipient Per 2 C.F.R... 2�, Cal OES is required to evaluate the <br />Grants risk of non-compliance with federal statutes, regulations, <br />Management and grant terms and conditions posed by each <br />Assessment Subrecipient of pass -through funding. The Subrecipient <br />Grants Management Assessment contains questions <br />related to a Subrecipient organization's experience in the <br />management of federal grant awards. It is used to <br />determine, and provide, an appropriate level of technical <br />assistance, training, and grant oversight to Subrecipients <br />during the subaward. The questionnaire must be <br />completed and returned with the Subrecipient's grant <br />application. <br />Application Indirect Cost Rate Agreement - If claiming indirect costs at <br />Attachments a federally -approved rate, please provide a copy of the <br />approved Indirect Cost Rate Agreement. <br />Federal Funding Accountability and Transparency Act <br />(FFATA) Financial Disclosure - Use the FFATA Financial <br />Disclosure to provide information required by the Federal <br />Funding Accountability and Transparency Act of 2006. <br />14 <br />
