· Provide input to CHCADA's service team regarding the service mix and intensity of <br /> services required by each youth; <br /> <br />· Make logistical arrangements associated with the provision of services to youth by <br /> other Network members, i.e., transportation, service location, etc.; <br /> <br />· Maintain inventories of materials and items needed for service delivery; <br /> <br />· Serve as a counselor; and <br /> <br />· Complete other duties as requested by CHCADA's Project Director, WIA, and/or the <br /> youth and his/her family. <br /> <br /> The Project Manager for this position will be Ms. Maureen K. Amaya. The Project <br />Manager's qualifications include each item listed in Figure 6 below. <br /> <br /> Figure 6: Required Qualifications of Project Manager <br />· :. An Associate's Degree in a Social Services area; <br />· :- Two or more years in a youth program; <br />· :. One or more years of experience with at-risk youth; <br />· :- Two or more years of experience as a youth counselor; <br />· :. Bilingual/Bicultural in English and Spanish; and <br />· :. Skilled in the use of Microsoft EXCEL and/or another data mana~lement program. <br /> <br />Task 2: Aid in the Collection, Maintenance, Storage, Retrieval, and Reporting <br /> of Applicable Data to WIA <br /> With the support of the entire counseling team, the Project Manager will also <br />assume another important duty - that of serving as Data Management Coordinator for <br />all participants who receive the WIA ten core services from CHCADA and/or other <br />Youth Network members. In the role of Data Management Coordinator, the Project <br />Manager will perform all duties and responsibilities listed in Figure 7 below. <br /> <br /> Figure 7: Data Management Duties of the Project Manager <br />The Project Manager will also: <br /> <br />· Ensure that CHCADA reserves copies of all files for all youth admitted to the program; <br /> <br />· Complete all CHCADA Intake Forms for the participants and enter these into the juveniles' <br /> files; <br /> <br />· Create document control numbers for each file and related alphanumerical coded control <br /> numbers for each document in each file; <br /> <br />· Inventory youth files monthly to ensure completeness; <br /> <br />· Maintain logs of all CHCADA staff visits service contacts; <br /> <br />CHCADA's Response to RFP# PY2002 Page 15 <br /> <br /> <br />
