· Create a record of all files and their documents in a Microsoft Access database and upload <br /> softcopy with attachments to CHCADA's file repository for the youth; <br /> <br />· Recall and/or verify all outstanding hard copy files; <br /> <br />· Maintain hard copy files for the participants arranged in alphanumeric sequence at <br /> CHCADA's administrative site for the program; <br /> <br />· Eliminate or avoid duplicate control numbers; <br /> <br />· Perform periodic audits to ensure the accuracy of each file relative to the softcopy repository; <br /> <br />· Maintain distribution logs for all records retrieved; <br /> <br />· Handle, store, protect and control documentation throughout the life cycle of the program; <br /> and <br /> <br />· Other duties as assigned by the Project Director. <br /> <br />Task 3: Collaborate With Youth Provider Network <br /> In addition to managing the files of participants, the Project Manager will maintain <br />data on the number of participants, entry and exit date from the program, services <br />received, problems encountered, problem resolution strategies, and on other areas. <br />CHCADA will utilize data collection forms required by the Youth Provider Network <br />and/or customized instruments designed by CHCADA. <br /> <br />Task 4: Establish a Process that Ensures the Timely Submission of Case and <br /> Activity/Service Data from other Network Providers <br /> <br /> All internal CHCADA service providers will provide weekly data on the services <br />and service units provided as a prerequisite to the receipt of paychecks. Services <br />provided by other Network members will also be closely tracked. CHCADA's <br />Counselor/Case Manager, with the support of the other staff, will schedule all service <br />appointments. These appointments will be logged. We will then call the provider to <br />confirm that the participant and/or his/her family kept the appointment. This information <br />will be entered into a Service Data Form. Additionally, CHCADA will give multiple copies <br />of the Service Data Form to all Network providers to whom the youth will be regularly <br />referred. The form will be faxed to CHCADA's office each time youth and/or their family <br />receive services. <br /> <br />CHCADA's Response to RFP# PY2002 Page 16 : <br /> <br /> <br />
