10.2 <br /> <br />death, and at the rate of pay effective on the date of death. <br /> <br />At his or her option, an employee may convert any lump sum payment <br />provided herein into health insurance premiums, to the extent necessary to <br />provide the employee and his or her designated eligible dependents, if any, <br />with benefits under the health insurance program maintained by the City. <br />The City's obligation to pay such premium shall terminate when the sum of <br />premiums paid equals one hundred fifty percent (150%) of the amount of the <br />lump sum payment that the employee would have received for unused sick <br />leave benefits had this option not been elected. <br /> <br />Premiums will first be paid out of the lump sum amount contributed by the <br />employee and then out of the amount contributed by the City. If the retired <br />~brmer employee dies before exhausting the full amount of his or her lump <br />sum contribution, the unused portion of such contribution shall be recovered <br />to cash and distributed to the retiree's designated beneficiary. <br /> <br />Except in cases of disability retirement, upon nondisciplinary termination of <br />employment after fifteen (15) years of cumulative full-time service with the <br />City, each qualified employee shall be entitled to payment for two-thirds (2/3) <br />of the total sick leave benefit credited to his or her account upon the effective <br />date of such termination to be applied toward employee and his or her <br />designated eligible dependents' health insurance premiums under the health <br />insurance program maintained by the City. <br /> <br />Bereavement Leave. An employee shall be granted up to three (3) days leave without loss <br />of pay in case of death ora member of the employee's immediate family. For purposes of <br />this Section only, a "day" shall be defined as the number of hours of work that an employee <br />is required to work according to his/her specific workday schedule. Such leave is designated <br />as bereavement leave. "Immediate family" as used in this section is limited to: <br /> <br />A. Any relative by blood or marriage who is a member of the employee's household; <br /> <br />A parent, parent-in-law, spouse, child, brother, sister, grandparent or grandchild of <br />the employee, regardless of residence; <br /> <br />Any other relative of the employee by blood or marriage where it can be established <br />by the employee that as a result of such relative's death, the employee's presence is <br />required. <br /> <br />10.3 Military Leave. <br /> <br />A.
