EXHIBIT C <br />STATEMENT OF WORK <br />Scope of Work. City hereby engages Provider as an independent contractor <br />to perform the following described work, and Provider hereby agrees to <br />perform said work upon the terms and conditions here -in -after set forth. <br />A. Specifically, Provider shall perform the following services: <br />Provider shall recruit their own participants within a reasonable <br />amount of time to allow for full delivery of program services as <br />specified in the Work Plan. <br />2. Provider shall assess each participant using a standardized <br />assessment tool within thirty (30) days of enrollment. If <br />assessment supports a Basic Skills deficiency, reading and math <br />must be the first service to be delivered to participant. <br />3. Provider shall identify through prescribed assessment which of <br />the ten elements as noted below will be prescribed for each <br />participant. Chosen elements will be noted in each participant's <br />Individual Service Strategy (ISS). Services needed but not <br />provided by Provider must be coordinated with the Service <br />Navigator and other youth partners. <br />The ten WIA elements are: <br />a. Tutoring, study skills and instruction leading to <br />completion of secondary school, including dropout <br />prevention strategies; <br />b. Alternative secondary school services, as appropriate; <br />Summer employment opportunities directly linked to <br />academic and occupational learning; <br />C. Paid and unpaid work experiences including OJT, job <br />shadowing and internships as appropriate; <br />d. Occupational skills training as appropriate; <br />e. Leadership development opportunities, which may <br />include community service and peer- centered activities <br />encouraging responsibility and other positive social <br />behaviors during non - school hours as appropriate; <br />
