0 <br />a. The name, address, telephone number and facsimile number of the prime contact for <br />this proposal. <br />b. The location /s from where the Vendor would provide services to the City. <br />c. A summary description of the organization, ownership and major product or service lines <br />of the Proposer's firm, the number of years the Proposer has been in the temporary <br />contract service personnel and consulting services business and any other information <br />the Proposer feels is relevant in helping the City gauge the Proposer's business stability, <br />overall experience and ability to perform these services. <br />d. A sample (no more than four) of the styles of resumes provided to clients. This will allow <br />the City to assess the readability of the formats and the usefulness of contents of the <br />resumes the City could expect from the Proposer. This is not to rate potential <br />candidates. <br />e. Identification of the types of technical specialties for which the Proposer provides <br />temporary placements. <br />f. Any unique qualifications and /or experience which distinguish the Proposer's ability to <br />provide temporary employees in the various technical specialties. <br />For example, a Proposer may provide 24 hour emergency placements, conduct in -depth <br />reference checking, conduct technical qualifying testing, use technical screeners to <br />evaluate candidates' skills, and track client satisfaction with candidates' performance, <br />etc. These are examples only and not mandatory services or qualifications. <br />To help the City assess the specialized qualifications of the Proposer to support the separate <br />categories of interest to the City, submit the following materials for each of the categories of <br />technical specialties being proposed: <br />g. Number, skill sets, and experience profile of the employees for possible consideration. <br />h. References. Proposer must supply a minimum of three (3) commercial firms or <br />government agencies that will attest to the Proposer's ability (in the last three years) to <br />provide services similar in scope and skills to those outlined in this proposal. <br />For each reference, provide the following information: <br />• The client organization's name, mailing address, contact name and telephone <br />number. <br />• The approximate period during which the services were provided. <br />• A summary of the type, quantity, and duration of placements. <br />2.4 Price Proposal <br />A price proposal is required for the temporary contract service persons outlined in the <br />Proposer's technical services and consulting proposal. In submitting proposals, Proposers are <br />asked to keep copies of the price proposal physically separate from the <br />other elements of their proposal in order to help the City conduct the technical <br />November 2001 <br />
